2014/6/3~2013/6/6

1. tdcst 20. tdcst21進行中。
2.Screening : 
   (a)exclude one OPD Hemorrhage potential(unwilling)。
   (b)exclude the old 2011 infarction potential(unwilling), one 2013 infarction potential(unwilling),one infarction still hold because of physical condition。
   (c) 5月case screen共26 cases,7 hold because of age and onset time,and will be
       discussed next week,others excluded.
3. tdcst16 3m再評待key。
4. stroke registry人數統計&圖done。

2014/5/26~28,5/30

1. tdcst 20. tdcst21進行中。
2. tdcst 21 MRI scan data 已轉已燒，QPI已燒。
3. tdcst16 3m再評已完成。
4.下周續問infarction and hemorrhage potential & start 5月case screen。
5. tdcst12 女兒本周仍busy。

2014/5/19~2014/5/23

1. tdcst 20進行中 . tdcst21本周開始。
2. tdcst 21 已進行MRI scan，data 待轉待燒。
3. tdcst 15 3m 追蹤評估已登。
4. 期中報告-篩選個案流程圖已完成。
5. tdcst16改約5/27。
6. (a)2011~2013 old list hemorrhage 2位no data者已check紙本，一位stage6 exclude，一位表示要考慮，但聽起來stage可能已高，需再連絡。
(b)Hemorrhage potentials from new list 共3位，一位六月回國可再問，一位上周已確認因交通問題無意願，一位本周表示需討論，個案無意願但太太有意願。

2014/5/12~2014/5/16

1. tdcst 20 進行中。
2. tdcst 15 3-month追蹤評估已完成。
3. tdcst 19 前後測評估報告已寄。
4. tdcst12 又再連絡，家屬表示需時間找人帶個案來，需再連絡 。
5. tdcst 21 下周二scan。

2014/5/5~2014/5/9

1. tdcst 20 進行中。
2. tdcst 19 前後測評估報告已寫已改。
3. tdcst19 已進行後MRI scan，data已轉已燒，QPI已給。
4. tdcst 15 約定於5/16追蹤，tdcst16 於5/21追蹤，tdcst12 家屬一直忙碌難約 。
5. tdcst 21 本周初評，資料已登，可收RCT，5/20 scan後開始。
6. 從舊名單找出 hemorrhage potential  total 21位，sceen for data及onset day中。Hemorrhage potentials from 新名單，一位六月中可收，一位需確認手術 and 意願。
7.  4月份case下周待screen。
8. 治療後主觀感覺評估內容。

2014/4/28~2014/5/2

1. tdcst 19，tdcst 20 進行中。
2. tdcst20本周結束，後評已評完，資料已登，評估報告待寫。
3. tdcst 18 MRI data已轉已燒。
4. 約 tdcst 15 於5/16追蹤 。
5. tdcst18治療前後比較報告已給
6.目前hemorrhage potential for May total 3: 已詢問2位，其中1位有意願但準備出國6/13回來，另一位無法配合來三天，待電話詢問1位 check stenting and 意願。
7.  目前infarction hold case for June MRI scan : 1位 old 2011 infarction potential check動作&意願 ；1位2013 check motion/carpal tunnel syndrome/意願 ；2位時間未滿hold；1位身體狀況較差持續追蹤。
8. 報帳list check完成。
9. 下周start screen 4月份case，keep finding hemorrhage case。
10. tdcst 19下周進行MRI。
11.想治療後主觀感覺評估內容。
